Improper Request Off

Failure to include a reason

This request off has been denied because it is lacking a reason in the request off request. Every request off requires a written reason to be included in the request. Please resubmit with the reason included in a timely fashion.

Failure to call ahead (less than 48 hours)

When a request off is less than 48 hours ahead, a phone call to your reporting manager is required prior to requesting off. All attendance policies are available at https://team.godlovesbrookline.com/mw/index.php?title=Staff_Attendance_Policy. This request is now unplanned or unexcused and must be resubmitted. Discipline actions may occur. Immediately speak with your reporting manager.

Failure to include Time Requested Off

The request off was for a partial day but was missing the actual times of day needing to be out of the facility. Please resubmit and include in the reason the times needed off.

The Day Requested off is Unpermitted To Be Requested Off

The day you have requested off is a day that no request offs are permitted. Blocked days are included on the Staff Attendance Policy (https://team.godlovesbrookline.com/mw/index.php?title=Staff_Attendance_Policy). If this is a medical emergency, Paid Sick Leave Request must be utilized for this day. If you think this is a true exception, please contact Pastor Lance directly via. phone as only he can approve this request off.
