DHS Required Contents of Child Files

Child Files must have the following according to DHS regulations 123, 181, and 182. All previous papers should ALWAYS be kept. For example:

  • an expired health report for a child who has been enrolled for 18 months still shows we had it within 60 days of enrollment
  • an original ECS to show previous pickup individuals, etc.
  1. Complete Emergency Contact Sheet
  2. Health Report less than 12 months old within 60 days of start
    1. more than vaccine record - full report
    2. flu shot less than 12 months old, or note for abstaining
  3. Enrollment Packet paper, especially:
    1. Emergency Care/First Aid
    2. Transportation
    3. Walking Trips
    4. Water Activities
    5. Hand Sanitizer
  4. Agreement (contract) with enrollment date and signatures
  5. "6 month file check" signatures
  6. Child Service Report within 60 days of start and every 6 months after
  7. Dietary and medication permissions where applicable (medicine permissions may be kept with meds instead of in file drawer)
