DHS Required Contents of Child Files
Appearance
Child Files must have the following according to DHS regulations 123, 181, and 182. All previous papers should ALWAYS be kept. For example:
- an expired health report for a child who has been enrolled for 18 months still shows we had it within 60 days of enrollment
- an original ECS to show previous pickup individuals, etc.
- Complete Emergency Contact Sheet
- Health Report less than 12 months old within 60 days of start
- more than vaccine record - full report
- flu shot less than 12 months old, or note for abstaining
- Enrollment Packet paper, especially:
- Emergency Care/First Aid
- Transportation
- Walking Trips
- Water Activities
- Hand Sanitizer
- Agreement (contract) with enrollment date and signatures
- "6 month file check" signatures
- Child Service Report within 60 days of start and every 6 months after
- Dietary and medication permissions where applicable (medicine permissions may be kept with meds instead of in file drawer)
