Failure to include a reason

This request off has been denied because it is lacking a reason in the request off request. Every request off requires a written reason to be included in the request. Please resubmit with the reason included in a timely fashion.

Failure to call ahead (less than 48 hours)

When a request off is less than 48 hours ahead, a phone call to your reporting manager is required prior to requesting off. Discipline actions may occur. Immediately speak with your reporting manager.
