Sprouts Volunteer Requirements

2. Child Abuse: https://www.compass.state.pa.us/cwis/public/home (you need to make a login first if you don't have one. If you have done volunteer background checks, you can use the same login, just make sure you chose the one for EMPLOYMENT for the new check.)

3. FBI Fingerprinting: https://www.identogo.com/services/history-check (you register and set up an appt, then go get fingerprints.) - You are being fingerprinted under code: 1KG738

4. NSOR: https://www.dhs.pa.gov/KeepKidsSafe/Clearances/Pages/National-Sex-Offender-Registry.aspx The pdf application is a little ways down the page, and directions for emailing it in are on the form itself.

5. Health Report with TB test: https://www.dhs.pa.gov/docs/For-Providers/Documents/Child%20Care%20Forms/CD-322%20-%20Staff%20Health%20Assessment.pdf (This can be done with your PCP or at Med Express (or anywhere that says they do "work physicals"). TB test can physical can technically be done separately, but sometimes doctors refuse to sign off on the physical without the TB first.

6. We need 2 non-family reference letters, which can be as simple as getting Carmen and Pastor Stephanie to write them. I'm going to ask them tomorrow.

7. I need a copy of your highest level of education - this can be unofficial transcripts, a copy of your diploma, etc.
