End of Day Documentation for School Age Lead

This list is designed to be a checklist/guide for the School Age teaching lead to ensure all paperwork and documentation is completed during end of day admin time. If any items are not completed by end of day, this must be clearly communicated promptly via slack to Administration. This is the exhaustive list for during the school year. Summer policies may differ.

Please note: Daily Requirements would be the only documentation expected to be completed by others in your absence unless otherwise communicated/planned (such as an extended absence). It will also be your responsibility to check that paperwork was completed upon your return within 1 business day, and that Administration is notified if follow up with coworkers is needed.

Daily Requirements (To be completed by the end of each day)

USDA Paperwork completed per their requirements:

  • Menu/Attendance Sheet fully completed and returned to the At Risk binder
  • Tick Sheets completed:
    • One for supper (or lunch on half days)
    • One for evening snack
  • Production Records completed:
    • One for supper (or lunch on half days)
    • One for evening snack
  • Google Form with meal counts submitted accurately, ensuring that unclaimed AM breakfasts are included

Reference sheets regarding meal patterns etc. are located in the At Risk Binder and can be reprinted by any Administrator with USDA portal access (PEARS). Additional trainings can be found on the Child Nutrition Toolbox web site.

Academic Documentation:

  • Daily Progress Notes for all children who were present in the designated binder on how the children performed during the daily activity per STARs requirements
  • Behavior Notes in Brightwheel for unsafe behaviors, repeated redirection, other behavioral concerns, demonstration of new milestone achievement, etc. per STARs requirements and Suspension policies
  • Messages with tags to coworkers about missing notes (if needed)
    • If something happened and was not witnessed by the lead or needs a second note for corroboration, the child's feed will be checked. If a note was not yet completed, a polite message with a tag of the coworker should be posted as a reminder in the class channel.

Weekly Requirements

  • Lesson Plan for following week ready for approval by Wednesdays at 4pm for Administration (a Group Supervisor) to approve per DHS requirements
  • Lesson Plan for the upcoming week shared and posted in the Front Classroom, bulletin board, and in the binder by Friday afternoon per STARs and DHS Requirements
  • Checking that the menu for the upcoming week has been posted in classrooms, and sending a Slack message with appropriate tags if it has not been done per DHS requirements

Monthly Requirements

  • Lesson/Learning Reflection Sheet completed and archived in the back of the Progress Notes Binder per STARs requirements

Ongoing / As Needed

  • Screeners completed within required timelines per STARs and DHS requirements
  • Assessments completed within required timelines per STARs and DHS requirements
  • Family Meeting Notes (including Parent-Teacher Conferences) documented promptly per STARs and DHS requirements
    • Please note, this is completing documentation and is not the meetings themselves that are conducted at other times
  • At lease 5 Rainy Day Lessons prepared and accessible in the designated binder
  • Reviewing paperwork completed by coworkers in your absence within 1 business day of your return
