Family Referrals

Purpose

To ensure that all families enrolled in our program have access to the social, mental health, educational, wellness, and medical services they may need, Brookline Sprouts at Tree of Life Open Bible Church. This policy supports our commitment to the holistic well-being of every child and family we serve.

Policy Statement

Brookline Sprouts recognizes that families may occasionally need additional support beyond the scope of our program. Staff are responsible for identifying and connecting families to appropriate community resources in a respectful, confidential, and supportive manner.

Procedures

  1. Identification of Need
    • Staff members who observe or learn of a family’s potential need for assistance (e.g., food insecurity, clothing needs, counseling, medical care, financial hardship, or educational support) will document the concern and discuss it with the Program Director or Assistant Director.
    • Conversations with families will always be conducted privately and with sensitivity.
  2. Referral Process
    • The Director or designee will provide families with information and referrals to trusted community partners, which may include:
      • Brookline Christian Food Pantry – for food assistance.
      • Brookline Boutique (Free Clothing Room) – for clothing.
      • 211 – for statewide access to social and human services.
      • Tree of Life Recovery Ministry – for substance use and recovery support.
      • Local Health Providers or Mental Health Services – as needed for wellness or behavioral health.
    • Families may receive written information, contact cards, or direct introductions (with consent) to community partners.
  3. Follow-Up
    • Staff may follow up with families, as appropriate, to ensure they were able to connect with the referred resource and to offer additional support if needed.
    • Documentation of the referral and any follow-up will be kept confidential and stored securely.
  4. Confidentiality
    • All discussions and referrals will be handled with the utmost confidentiality. Information will only be shared with family consent and in accordance with applicable privacy regulations.

Staff Training

All staff will receive orientation on this policy upon onboarding and annual reminders about available community resources and the appropriate steps for making referrals.

Review and Updates

This policy will be reviewed annually by program leadership to ensure accuracy and the ongoing effectiveness of community partnerships.
