SICK LEAVE POLICY

Tree of Life Open Bible Church has created this Sick Leave Policy in an effort to provide eligible employees with time off work as a result of a disability or in the event of illness for certain family members. This Policy is also intended to comply with any statutory requirements for paid sick time.

Eligibility

All employees who work in the City of Pittsburgh for at least forty (40) hours or more within a year are eligible for paid sick leave. However, employees are not eligible to take paid sick time until they have worked for the Company for ninety (90) days from the date of hire.

Accrual/Usage

Eligible employees will accrue one (1) hour of sick time for every forty (40) hours worked up to a maximum accrual of forty (40) hours or five (5) days, whichever is greater, per calendar year.

Accrued, unused time under this policy will carry over each year up to a maximum accrual of forty (40) hours or five (5) days, whichever is greater. Employees are required to use sick time in increments of no less than one (1) hour, up to a maximum of forty (40) hours or five (5) days, whichever is greater, per calendar year.

Non-exempt employees will accrue sick time based on total hours worked, including overtime. Exempt employees are presumed to work forty (40) hours per workweek for purposes of sick time accrual. If their normal workweek is less than forty (40) hours, accrual will be based on their normal workweek.

Leave under this Policy may be used in connection with the diagnosis, care or treatment of an existing health condition for employee or covered family member, or the preventive care of an employee or employee’s covered family member. “Family member” for purposes of this policy includes spouses, registered domestic partners, children (regardless of age), foster children, parents (including step-parents and parents-in-law), grandparents and siblings.

Leave under this policy may also be used for employees who are the victims of domestic violence, sexual assault or stalking. Also, leave under this policy may run concurrently with leave taken under other applicable policies as well as under local, state or federal law.

Notice Requirements

If the need for paid leave is foreseeable, employees shall request time off with his/her manager in advance. If the need is not foreseeable, and you are sick and unable to come to work, you must personally notify your manager, as soon as possible, but no later than three (3) hours before the start of your shift. For each day you expect to be absent, you must notify your manager at least three (3) hours before the start of your shift. If you become sick during the day, you must notify your manager prior to leaving the workplace. In the event of a medical emergency and you are unable to contact your manager, you may have someone call in the absence on your behalf.

Proper notification is a verbal conversation between the employee and the employee’s manager. Voicemails, slack messages, text messages or any other communication other than a two-way verbal conversation between the employee and the employee’s manager will not be accepted.

Employees who miss more than two (2) consecutive days, or who exhaust their sick leave may be required to present a doctor's note upon returning to work. An absence of three (3) days without notifying your manager will be deemed as job abandonment.

Unused time under this policy is not paid out at the time of separation from employment.
